Prepare a privileged psychotherapy note that you would use to document your impressions of therapeutic progress/therapy sessions for your client family from the below information/scenario
39-year-old A.A male: “My family wants me here for therapy because of my drinking.” The Client presents in the clinic with Hx of alcohol abuse and has had two DUI in the past two years. The Client appears guarded and affect appropriate to speech content. The Client did not present with any memory or intellectual deficit and was able to provide background information about his situation. The Client is not on any medication, and none initiated at this time.
History of Present Illness: The Client lost his regular job a little bit over three years ago and has been working as a contract staff in a construction company. He informed the therapist that he has been doing odd jobs to augment with his present employment, which is not steady. The Client reports that not having a permanent job is very difficult for him to take care of his family of five. I crave alcohol and only drink anyway when I am stressed, which is a lot lately.” He reported drinking four bottles of beer daily with some “shots” of liquor and have been drinking for over three years.
Past Psychiatric History: The Client has a hx of alcohol abuse and has had two DUI in the past two years. J.D. is in denial about the extent of the alcohol abuse problem. Client has not sought treatment for his alcohol abuse and states “I did not think that was a problem.
Psychosocial History: Client is a welder. He has a high school diploma and attended two years of trade school. JD is married and lives with wife and their two daughters, ages 10 and 5. JD come from a large family while his wife is adopted and an only child. Client diverts energy from work, family, and social life and spends time with friends whom his wife calls his “drinking buddies” in order to drink. There is a history of substance use. The Client’s father (deceased)was an alcoholic for twenty years. The Client’s paternal grandfather (living: 85y/o) was an alcoholic but claims to be a social drinker now. Uses tobacco. The Client’s two younger brothers smoke marijuana and drink alcohol regularly. The Client’s paternal family is into some drug/alcohol use.
History of abuse/trauma: History of abuse/ trauma gives a therapist insight into the dynamics of the family. J.D. was verbally and emotionally abused as a child by his parents. J.D. because of the emotional abuse at home, especially when his father drinks attempted to run away from home. Being the oldest son, J.D., was expected to take up responsibilities at a young age.
