Mr. G is a 78 year old male with a diagnosis of Myasthenia Gravis. He has had the disease for 15-16 years. He has had several crises with an increased frequency in the last year. He lives alone. His daughter and son-in-law live in town and have two children ages 12 & 14.
He was admitted over the weekend with fever, cough and increased sputum production. His chest X-ray shows a right middle lobe pneumonia. He had a modified barium-swallowing test this morning. The speech pathology’s assessment indicates that he is an aspiration risk with all food consistencies and not safe for oral nutrition. The hospitalist has entered the room to discuss the insertion of a percutaneous endoscopic gastrostomy tube so he can receive enteral feedings and be discharged home with home health and IV antibiotic therapy. After the physician leaves, you find the patient crying. He tells you he is upset about the results of his test and the plans the doctor discussed with him. He tells you that he loves to eat and is not sure he wants to have the PEG tube and “eat” that way.
