For the writer: I work as the only Clinical Case Manager in my region for a Medical Insurance company for the Long Term Care (LTC) division. My workplace is an office, not a medical center (hospital, clinic, etc) I work by coordinating needed services for members at their houses, for example, Home Health Aid (for Personal Care, Homemaking Services, or Companion Care; Skilled Nursing Services, Respiratory / Occupational / Physical / Massage Therapy; Home Delivery Meals; Consumable Medical Supplies (pullups, diapers, under-pads, liners, sealing creams, gloves, masks, etc etc; Durable Medical Equipments (DME – Such, Ventilators, Cough assists, Urinary Catheters, tracheostomy supplies, colostomy supplies, etc etc). The services I provide are based in medical necessities, and require an ongoing follow up and coordination with a multidisciplinary team (physicians for prescriptions, medical providers for therapies, medical equipment providers, and home health aide providers). The membership for this type of LTC program are at greater risk For this assignment: Ever since this new situation with the COVID-19 started, the government indicated that all LTC Plans throughout the state must create a team lead by nurses to screen, follow up, and coordinate further services to members at higher risk of contracting this virus. In my region, for this LTC program, there are only two field nurses, the Transitional Nurse and myself, the Clinical Case Manager. It was required for us to design a new screening tool and divide members into three categories: 1,2 and 3. To make a decision, we needed to consider comorbid conditions (bedbound, tracheostomy, oxygen-dependent, chronic kidney disease with hemodialysis, chronic heart failure, asthma, COPD, diabetes mellitus), hospitalization history within the last year, chronic base conditions, and support system (caregivers, neighbors, family members, providers). Members in category 1 are those at the highest risk among our membership, and we decide to follow up on these members weekly. For these members, it is necessary to ask each week the same questions… Have you developed any exacerbation of your symptoms within the last week? Have you had any fever or visited any hospital within the last week? How is your breathing? Is it harder than usual? Do you have any other concerns? Are you still receiving services through this LTC program? Are you satisfied with the services? If the answer is ‘no’, then we just call next week, but if the answer is ‘yes’ we need to coordinate immediate services with the primary care provider (PCP) to address the new symptoms, or request hospitalization if severe symptoms are developing (such respiratory distress). We also need to complete weekly reports to the state, indicating how these members are being followed weekly. Members in category 2 are less intensive, and we need to contact them every other week. We follow the same set of questions and the same protocol if there is a ‘yes’ answer. For this project, we don’t have to follow up on members in category 3. These members are regularly taken care of by their Case Managers, but they don’t meet the necessary criteria to be included in a more intensive follow-up. This project has put much pressure on the Transitional Nurse and me because of its importance and weekly requirements. It’s much work we have to accommodate in an already busy schedule, and we have to comply with it as part of our performance measures. Since there is no near end to this virus situation, this it’s something we are still doing and will continue to do (please elaborate here).
