You are conducting a history and physical assessment on a 75-year-old male client. The subjective information related to the skin and nails indicates the following: The client reports that he has started to notice over time that whenever he bumps his arms or hands against something he seems to get purple areas under his skin that do not look very nice. The client also tells you that he noticed that his nails have changed shape, and they look like they have a “bubble” on the base. You also learn that he is a long-time cigarette smoker of two packs per day. Objective findings of the skin and nails confirm a few purple macular areas on the hands and forearms bilaterally. You perform the Schamroth technique where you observe that the nails no longer can touch at the tips to form a diamond-shaped opening and they are pointing outward. Step 2 Respond to the discussion questions.
