Final Case Presentation
For the Unit 9 Assignment, you will synthesize your understanding of the theories, best practices, and current trends in human services to create a case presentation that would be used during a case conference/staffing meeting in an agency. While most of these meetings are informal, many agencies/organizations use a more formal approach. To prepare for this future job responsibility and leadership role, you will create a paper that could be used during one of these meetings. Use the paper structure below to organize the facts and recommendations for your hypothetical client.
Begin by deciding the type of agency and job that interests you, and imagine you are a professional working at this. Describe the Social Worker supervisors roles and responsibilities as well as the NOHS code of ethics that pertain to this case (Note: The career planning exercises you completed for the Units 1–8 Learning Journal will help you with this part of the Assignment.).
Next, create a hypothetical client or use one from a prior exercise in our class. Follow the suggested structure below for writing your paper. Add relevant details and leave out minor ones that are not relevant to the client’s progress. While you will not be required to write papers for these meetings, you will be required to report this type of information and to be organized and prepared. This exercise will assist you in developing a case presentation for when you really will be making them.
Suggested Organization of Paper: use sub-headings to organize your information:
Type of agency and your role/job description (no sub-heading)
Client name and demographics (no sub-heading)
Presenting problem (sub-heading): What was the reason for the client enrolling in service at your agency? Be specific about the problem, but do not add many extra details by “telling the client’s story;” just write a few sentences about the problem as the client perceived it.
Goal (sub-heading): What are the client’s main intervention/treatment goals? Be specific and add as many as you can think of that would relate to the full scope of services that your agency might provide. However, remember that clients may want to limit their goals to three goals at a time or fewer so it is important to prioritize goals. Goals should adhere to the SMART criteria.
Needs (sub-heading): What other needs does your client have that others in your agency or outside agencies might be able to provide or assist in making referrals?
Crisis (sub-heading): Probably, your client is not currently in crisis, so you would simply report that the client is stabilized and there are no ongoing crises for the moment. If your client is in the middle of a crisis, you would briefly describe it, only giving facts that are relevant.
Recommendations (sub-heading): This is the part of the paper (meeting) where you provide your professional opinion about the continued direction of the client’s progress, ideas for future goals, planning or strategies for helping the client, resource referrals, and any other suggestions that you have to advance your client’s care. You will spend time using information from your past classes to support your prioritized recommendations, so make this one of the longest sections of your paper.
*This paper should be a minimum of 8-10 pages or a minimum of 2000 words, not including the title and reference page, with a minimum of five scholarly, academic references in APA formatting. ***Below is the backstory to “client Daphne” please use the below scenario for paper, the agency choice will be Child Protective Services (CPS) the over all role will be a Social Worker Supervisor***
Daphne is a 14 year old girl who lives in inner-city Detroit. Daphne has three younger siblings that live in the house with she and her mother Cora in a small two bedroom apartment in the projects. Daphne’s father Alvin has an extended history of physical violence on Daphne’s mom along with drug abuse issues. Alvin was recently arrested for blackening Cora’s eye and throwing her down a flight of stairs in a heated argument. The argument stemmed from Alvin stealing rent money that was located in Cora’s purse. In the event of falling down the stairs, Cora broke her arm and took a pretty hard hit to the back of her head. While receiving emergency room services at the hospital Cora reported the incident to authorities. While at the hospital, Cora’s younger children were routinely checked for signs of child abuse and informed the doctor and hospital Social Worker that they were scared to return back to the home with Cora for fear of their father hurting them “really-really bad.” During the initial interview the kids stated that they did not attend school on a regular basis and were often kept home from school because of visible bruising. They also said that they have very little if any food in the house to eat, they mentioned that they rarely took baths or had clean clothes to wear, they stated that they currently slept in one room on the floor with one blanket between them. The youngest child who was about three or four had no shoes on their feet during the time in emergency or no jacket. When speaking to the hospital Social Worker 14 year old Daphne reveals a family secret; the secret is that she has been molested by her father for the last four years and that she is now two months pregnant with his child. She makes it clear to the worker that she has not been sexually active with anyone else and there is no possibility that the child could be another mans. Daphne states that her mother has known of the molestation since Thanksgiving morning of last year when she walked in the bathroom while Daphne was performing sexual acts on him. Daphne states her mother has mentioned a word of the incident and has since blamed her for the additional stress and turmoil in the household. Daphne, has decided that she would like to have an abortion and get out the house and move with her mother parents as soon as possible. She would like her younger siblings to be removed from the home for fear of what could happen to them in the future. As the worker is gathering information from the family Cora confesses to losing her job three months ago and has been supplementing her income by stripping at a local strip club and prostituting in order to make money to pay the bills. Cora admits that this has made things with Alvin more intense being that he does not like the idea of her sleeping with and entertaining other men. Cora explains she hates working for the club and escort service but does not have many job skills that would allow her lead way into the career force. After bringing up all the allegations to Cora, she admits she has not been a perfect mother and times has been hard but she would like some help moving forward.
Daphne comes to Child Protective Services as a referral from a hospital report derived from a domestic violence incident that happened between both her parents that caused physical harm. Because, the children were in the home during the incident CPS has been notified and an investigation has been opened. Multiple children have made allegations of neglect, abuse and sexual misconduct to the hospitals’ Emergency Room Social Worker. As, a Human Services Professional you are a mandated reporter or legally required to report suspicion of child neglect, child abuse, or other unsafe parenting practices in a child’s home (California Department of Education, 2020). The purpose of CPS is to minimize the abuse in a child’s home and create supportive services and or resources to strengthen the families overall need. As the Social Worker on the case, I will be focusing on rehoming the children with a family member or finding adequate foster care for all four children, hopefully a home where they can remain together. The next concern will be bringing Daphne into a health site for CPS and confirm that she is in-fact pregnant. If she is found to be pregnant we can proceed with getting consent for the abortion process and the filing of criminal charges against the father (Alvin). During working with the family, I’d like to begin play therapy with the younger children to see if other trauma arises and provide some trauma counseling to Daphne for the molestation by her father and the decline and resentment in her relationship with her mother. At this time, I’d like to allow the kids 30 days before they see their mother in order to settle in to their new living situation and to begin the counseling services. In this time, I will refer Cora to Human Services for supplemental cash benefits, food stamps, MediCal, job training services, or the enrollment in a vocational school program in attempt to help her get on her feet and be able to provide for herself and her children in the event they are returned to her care. At this time, I will request emergency Section 8/HUD Housing so that she can get into a lower priced rental unit and get help with monthly rental cost. I’d like to enroll Cora in parenting classes and get her involved with a family support advocate or support group with other mothers/fathers that have been through the CPS process and knows the reunification process. Lastly, I will refer her to the salvation army to help her out with basic household items like, couches, beds, blankets and other essential household items to get her house ready and suitable for the children. Ethically, I will work towards building on the family’s strengths and utilizing community resources, respect my client’s confidentiality and their personal information, disregard all bias and individual transferences that may impact treatment decisions, and be careful in the usage of technology or sending of information through personal email/social media or any other internet usage (NOHS, 2020). One of the most important ethical standards is standard two which is issuing the client an Informed Consent Form at the beginning of treatment. This form is a complete overview of treatment and services, it also provides consent to other family members or close relatives that can be included in gaining information of the treatment plan (NOHS, 2020). This conform can be a protective affidavit in the event of misconduct or malpractice issues. In reviewing statistics for this demographic, it is reported that according to the United States Census Bureau 38.6% of people living in Detroit, Michigan are living in Poverty (United States Census, 2020). Detroit is 78.6% Black/African American with 14.6% of persons 25+ graduated with a Bachelor’s Degree (United States Census, 2020). These stats don’t provide a positive feedback for level of success in the area. The overall goal is to get all parts of the family working and actively building together for a stronger family unit.
The Strength-Based Theory focuses on a client’s strengths and determination (Stoerkel, 2020). This theory applies all the positive attributes currently going on in a client’s daily lives. For instance, what’s currently working in their current situation. For example, is Alvin currently attending outpatient drug treatment, or Cora actively seeking work outside the strip club or her escort position. Strength-Based Theory solely identifies those things that can be beneficial to the client’s treatment plan/process opposed to impacting in a negative way. Positive reinforcement in this theory, allows the client to see themselves as valuable to the process. This theory examines demographic, community, cultural, and socio economic constraints as well as active support systems including family/friends (Stoerkel, 2020). When used in a counseling setting, SBT allows the client to identify reasonable expectations that are attainable and can be met within a certain timeframe (Stoerkel, 2020). This theory see’s people as resilient instead of broken which adds confidence and optimism that promotes rebuilding instead of failing. In correlation with Maslow’s Hierarchy of Needs which defines the human need in five pyramid tiers, the lowest tiers being the most imperative for ultimate survival and the higher levels being achieved after the lower levels have been reached (McLeod, 2020). In Daphne’s case, she and her siblings basic level of needs have not been met, those bottom tier needs consist of food, water, rest, and warmth (McLeod, 2020). Cora fails to provide the children with food, water, adequate sleeping arrangements and basic clothing to keep them warm. The next tier is defined as security and safety which has not been provided in Daphne’s house due to physical abuse and the allegations of molestation by her father (McLeod, 2020). The next three tiers build on Psychological needs and self-fulfillment needs which aid the client in intimate relationships, friendships, feelings of accomplishment and achieving personal goals and the client’s full potential (McLeod, 2020). A variety of obstacles have potential to stand in the way of this family like legal prosecution of the father and pending jail time for the crime committed on Daphne. The loss of both parents could cause a potential breakdown in emotional or academic performance and or the ability to focus and be present in their new living environment. Daphne could make her mind up and decide to give birth to her and her father’s child and attempt raising the baby on her own. Daphne’s mom Cora could decide that she’s not able or interested in obtaining vocational training or attend the suggested parenting classes. Other obstacles that could present themselves, is not being able to place all the children in the grand-parents care and having to separate the siblings in multiple foster care placements.
During the evaluation process, I will review the six month treatment plan and follow-up on how each intervention method worked. For instance, was Alvin fully prosecuted? Were the children placed with the grand parents or foster care? Has Daphne gotten an abortion? Did Cora receive benefits, and has she completed a vocational trade program? Has Cora received new items for her household, or obtained housing? Has the family spoken or received counseling together? Were there new abuse developments in the smaller kids play therapy sessions? In this time, I can focus on the outcome and what things worked. These outcomes can highlight programs that worked well within the community or local agencies. According to the NCBI, “A process evaluation documents how a program or intervention is executed and helps our understanding of the relationship between the elements of a program or intervention and its outcomes” (McCarthy, Dickson, Katz, Sciacca, & Chyun, 2015). Process evaluations can lead to additional treatment support and identify areas in which the family or certain individuals obtain help. This could be a time where Cora needs additional assistance with employment resources or help contacting and agency for resources.
In order to plan for the last session proper review with each participant in the treatment plan is needed in order to verify that all services were rendered according to what was originally specified (McCarthy, Dickson, Katz, Sciacca, & Chyun, 2015). During this time, considering all client feedback and concerns will be taken into consideration for additional treatment or services if needed. At this time, I can reflect with Daphne and make sure that she feels secure in moving forward. This is also a great time to assign additional tasks or small attainable goals. During this last meeting I’d like to see Cora visiting her children at the visitation center once a week in order to rebuild the bond. If the mother has not let relocated out of the area, I’d like her to research lower crime rate areas as well research jobs in another location. The overall goal in the last meeting is to assess how far the client has come and identify yourself as an additional support if needed down the line. This is the perfect push for the client to continue working hard or their treatment plan and continue pushing themselves and their families needs.
