Mrs SE is a 83 year old Arabic-speaking female with no known past psychiatric presentations, with Stage 2 progressive Parkinson’s Disease, who presented to ED from a nursing home with a psychotic episode. This essay will discuss the use of antipsychotics in a patient with Parkinson’s Disease and the clinical balance of managing psychotic symptoms and extra pyramidal side effects of antipsychotics.
Mrs SE was brought into the emergency department by ambulance from a nursing home following bouts of yelling and communicating to people that were not present. An Arabic-speaking Emergency physician identified that she was communicating with her deceased mother and referencing a “boy drowning” that had to be saved. She was very panicked and upset. This episode was recorded to not have any precipitating physical or emotional triggers and was her first ever recorded episodes of experiencing hallucinations or delusions.
