Go to https://www.google.com/forms/about/ and under “Personal” select “Go to Google Forms”
You will be asked to sign in to a Google linked account. If you have one, use it to access Google Forms. Otherwise, create a Gmail account to access the Google Forms application.
Under “Start a new form”, select “Blank”
Give your survey a title and description.
Then proceed to insert your questions one at a time.
Select “Multiple Choice” in the right-hand drop-down menu and then insert your five closed-ended answers to each of your questions.
You can have two sections to your survey – the first section can be called “Bio-Data” and records the 3 – 5 questions about your respondents’ background (i.e gender, age, race, occupation, income, etc.). The second section can be the substantive questions that relate to your research question. Come up with a title for this section that is appropriate (i.e. The Dating Survey). There is a floating side menu next to each question you are drafting with a number of icons – select the last icon to create a new section.
You are able to drag-and-drop questions in order to re-order the items. You can also add images and text, if appropriate.
The wheel icon on the top-right hand corner allows you to customize the settings for the survey.

D. Write your memo.
Your assignment should use APA format, including a cover sheet, page numbers, double‐spaced, and free of spelling and grammar errors. Do not write an abstract. This assignment must be formatted with 1-inch margins and use 12-point Times New Roman or Cambria font. Your name, the course code (LWS 225) and section number (01 or 04) must appear in the header. Save your paper repeatedly as you write it, in .doc or .docx format. Please read over your paper and correct errors before handing it in.
Project Sub-Sections
Write a memo including the following sections, with headings centered as shown. Write 3-5 sentences for each section below –Introduction, Data Collection, Results, Discussion. The word count for your memo will be around 750 words. Your two pdf copies of the Questions and Summary pages will be the appendices that follow this memo.
Introduction
(Paragraph 1) What is your research question? What were you trying to find out in this survey design assignment? Why should people be interested in this topic and your results?
Data collection
(Paragraph 2) What were your 3-5 substantive questions that contribute towards your Research Question? For each question, identify the five answer choices.
(Paragraph 3) Now begin a new paragraph. What were your 3-5 demographic questions? For each question, identify the five answer choices.
(Paragraph 4) In another new paragraph, define your concepts. For each of your concepts that have an ambiguous meaning, how did you define it for purposes of your project? (e.g., if you asked about “crime” or “exercise”, what is the definition of “crime” or “exercise” for purposes of your project?)
In the same paragraph, explain whether this a cross‐sectional or longitudinal study, and why?
Results
(Paragraphs 5 and 6) To present your results, identify the most popular and least popular answer given for each of your survey questions, along with the percentage of respondents who provided those answers. Do that for all substantive questions in one paragraph, then for all demographic questions in a second paragraph.
In these two paragraphs, don’t include your opinions or interpretations, just the results themselves. Write in full sentences, not a chart or graph or list. Do not submit the full set of results that Google Forms gave you.
Discussion
(Paragraph 7) First, what is your overall result – i.e., you explained in your Introduction what you were trying to find out, so what’s the answer?
Second, what are the implications of your results? I.e., what do they tell us? How would you answer if someone asked “so what” about the results you discovered in your research?
(Paragraph 8) Third, in a new paragraph, share at least two reflections on the survey you created. Did you find that anything in it limited its validity or reliability? If you could do it again and had more time, what would you do differently, and why? Or, if you think it was perfect, what made it work well?
